No Application Form Required

When you decide to apply to live at a Palm Lake Community, you simply need to contact our Customer Experience Support Team.
They will discuss your individual needs including personal information, details about your health and care needs, legal and financial information.
Palm Lake Care’s focus is ensuring our Communities are the best solution for your ongoing care needs. Our Customer Experience Team will guide you through the enquiry process and advise if we are able to assist you or if other solutions may be more suitable.

Provide supporting documents

To help us understand your care needs in more detail, we will ask you to provide clinical/care documents such as:

GP Health Summary – within the last 30 days and signed by the GP

Hospital Care Summary and medication charts – if a current patient.

Income and Assets Summary from Centrelink – for pensioners OR

Financial Summary from your accountant – for self-funded retirees

Enduring Power of Attorney (EPOA) or QCAT Orders: This confirms the person making decisions on the client’s behalf has the authority to do so.

Admission Suitability Review

Once we have received all the requested information, we commence our review. A member of our clinical team will call either the client or their delegate to discuss care needs.
The Customer Experience Team will keep you updated on progress towards a decision.
It’s important to note that the review process can be complex, we work closely with you to ensure that you feel supported and informed. We understand that this can be a confusing and emotional time, and we strive to make the process as seamless and stress-free as possible.

Organise a Pre-admission

Just like preparing for a hospital admission, out Pre-admission Phase requires you or your representative to ensure important clinical and non-clinical forms have been completed and returned prior to admission date. Our Customer Experience Team will guide you through what is needed and why.
Attendance at a Pre-admission Meeting to ensure all documents have been completed correctly and the clinical team can start preparing for the admission.
It’s all aimed at making sure the admission date is NOT delayed and admission day is smooth and as stress free as possible.

Move in day

On the day of admission, the Palm Lake Caring Community will welcome you to your new home and help you to settle in.
We will show you all the features of your room, introduce you to key people and other residents. Don’t worry- we know you probably won’t remember everything, so its ok to ask questions. Over the next few days, we will show you more areas and get you as involved as you would like to be.
There is lots of information including a Resident Handbook provided in your Welcome Pack.

We have 3 payment options

1

A lump sum Refundable Accommodation Deposit (RAD) or Contribution (RAC).
The RAD or RAC is a lump sum payment for accommodation in an aged care home.
This amount is fully refundable when you leave, less any amounts you have agreed to be deducted.

2

Daily Accommodation Payment (DAP) or Contribution (DAC) (Rental style/non-refundable).
Instead of a lump sum payment, you can choose to pay a daily fee based on the room price.
The DAP is calculated using a government-set interest rate- known as the Maximum Permissible Interest Rate (e.g., 8.15%).

3

Combination part lump sum (RAD) and part ongoing payments (DAP).
You can choose to pay part of the accommodation cost as a RAD and the rest as a DAP.
This can reduce your daily payments while still providing a refundable amount for the future.
