Complete application form

When you decide to apply to live at a Palm Lake Community, you will need to complete an application form, provided by our Customer Experience Team.

This form will ask for personal information, details about your health and care needs, legal and financial information.

Palm Lake Care’s focus is ensuring our Communities are the best solution for your ongoing care needs. Our Customer Experience Team will guide you through the enquiry process and advise if we are able to assist you or if other solutions may be more suitable.

Provide supporting documents

To help us understand your care needs in more detail, we will ask you to provide clinical/care documents such as:

GP Health Summary – within the last 30 days and signed by the GP

Hospital Care Summary and medication charts – if a current patient.

Application approval process

Once the application is submitted, we commence our review. A member of our clinical team will call either the client or their delegate to discuss care needs.

The Customer Experience Team will keep you updated on progress towards a decision.

It’s important to note that the application process can be complex, throughout the entire admission process, we work closely with you to ensure that you feel supported and informed. We understand that this can be a complex and emotional time, and we strive to make the process as seamless and stress-free as possible.

Organise a Pre-admission

The Pre-admission Meeting is an integral part of preparing for admission to aged care.

It enables us to complete any necessary documentation and commence the clinical assessment process.

It’s all aimed at making sure we are ready on admission day, and everything is in place.

Move in day

On the day of admission, our team will welcome you to our Palm Lake Caring Community and help you to settle into your new home.

Pricing

Maximum prices shown may not apply to you

Some people may only be required to pay the basic daily fee if their assessable assets are below the threshold and their only income is the standard aged pension.

Understanding the fees and charges associated with aged care is made easier by the knowledgeable Palm Lake Care Customer Experience Team, who will explain them in a simple yet comprehensive manner using our Quoting Tool.

You will also receive a current Pricing Guide.

We have 3 payment options

1

A lump sum Refundable Accommodation Deposit (RAD) or Contribution (RAC).

The RAD or RAC is a lump sum payment for accommodation in an aged care home.

This amount is fully refundable when you leave, less any amounts you have agreed to be deducted.

2

Daily Accommodation Payment (DAP) or Contribution (DAC) (Rental style/non-refundable).

Instead of a lump sum payment, you can choose to pay a daily fee based on the room price.

The DAP is calculated using a government-set interest rate- known as the Maximum Permissible Interest Rate.

3

Combination part lump sum (RAD) and part ongoing payments (DAP).

You can choose to pay part of the accommodation cost as a RAD and the rest as a DAP.

This can reduce your daily payments while still providing a refundable amount for the future.

I just want to thank each and every one of your staff for the love and care you have shown Mum and myself and my family during this year.

- Lorraine Keen

What wonderful staff you have. Every day I am so grateful that mum was accepted into your care.

- Lorraine, Shirley Hurley’s daughter

We are very grateful for everything that we received, and we feel that the people of Bundaberg need to know that there is excellent elderly care available.
